About the Department
Halton Healthcare is seeking Locum and Part-time Hospitalists to join the Department of Hospital Medicine. Hospitalists care for unaffiliated patients in acute medical, surgical, mental health, rehabilitation, and complex continuing care inpatient units. Working with a collegial team of Hospitalists, you will be part of a strong, collaborative interdisciplinary team, supported by approachable specialists with access to advanced diagnostic and therapeutic services. Hospitalists are not restricted to one site and may be required to support tri-site needs.
Responsibilities:
- Manage the care of assigned inpatients including recognition, problem identification, diagnosis, coordination of investigative and therapeutic interventions, goals of care identification, monitoring changes in health status, and determining medical stability
- Provide medical care consistent with legal and regulatory requirements, evidence-based best practices, patients’ diagnoses, condition, prognosis, and goals of care
- Support and demonstrate the values of Halton Healthcare: Compassion, Accountability, Respect
- Address patient care needs, visits, phone calls, and documentation in an appropriate and timely manner
- Identify circumstances in which palliative and/or end-of-life care may benefit the patient and family members
- Collaborate effectively with members of the interdisciplinary team in providing care based on understanding and valuing the general roles, responsibilities, and levels of knowledge and training for those of various disciplines
- Exhibit professional, respectful, and culturally sensitive behavior for working with patients, their families, and members of the interdisciplinary team
- Support transitions of care between hospital medicine providers and upon discharge or transfer of inpatients into the community or within the hospital
- Provide care that uses hospital and community resources effectively and efficiently
- Attend departmental and hospital quality meetings
- Develop a continuous professional development plan including an annual course focused on acute hospital medicine using relevant opportunities from professional organizations
- Maintain licensing requirements and in professional college certification programs
- Participate in on-call and scheduled clinical responsibilities
What We Offer
Part-time, weekend-only, summer locum, and nocturnist opportunities are available which offer flexible scheduling. Participation in hospital quality initiatives and committees is expected. As a teaching site for McMaster University, and Toronto Metropolitan University, the Hospitalist team provides teaching to family medicine residents, medical and physician assistant students, for which a faculty appointment can be arranged.
What We Are Looking
Qualifications:
- Licensed by the College of Physicians and Surgeons of Ontario or eligible for licensure
- Demonstrates clear interest and recent experience in acute hospital care
- Additional competency training or experience in Hospital Medicine preferred
- Knowledgeable in the Public Hospitals Act, 1990 and the legal and regulatory provincial requirements regarding consent, capacity, and substitute decision making
- Demonstrates ability to be adaptable, build trust, provide coaching and mentorship, facilitate change and quality improvement
- Demonstrates skills to effectively interact with staff, patients, and families by using appropriate strategies to address sensory, language, health literacy, cognitive, and other limitations
- Demonstrates ability to improve personal practice and patient outcomes by evaluating patient adverse events and outcomes
- Demonstrates skills to operate electronic medical records and use electronic mail (e-mail) effectively
- Demonstrates interest in participating in hospital quality improvement initiatives
- Demonstrates flexibility and collaboration, with the ability to work across our three hospital sites: Georgetown Hospital (GH), Milton District Hospital (MDH), and Oakville Trafalgar Memorial Hospital (OTMH)
